Our Goal
in Bramalea
To build healthy individuals and families, as well as a healthier community. To achieve this goal, we'll be working very closely with those we serve. Working in partnership with community members, and coordinating efforts with community based agencies, means we can maximize opportunities for improved health.
Primary
Health Care
Primary health care services are currently delivered at our Centre at 40 Finchgate Blvd. However, we are working hard to set up mobile cinics in other parts of Bramalea; so keep an eye out for a clinic near you! Please click here for more info on how to become a client.
Registered clients can access services from a team of health providers that includes:
- Nurse practitioners
- Family physicians
- Registered nurses
- Community health educators
- Medical office assistants
- Social workers
- Dietitians
- Physiotherapists
- Pre-natal care
- Post-natal
Who
Do We Serve?
We serve residents of Bramalea who identify with one or more of our priority populations:
- Youth
- Isolated Seniors
- Newcomers
- People who are homeless, under housed, or at risk of homelessness
- People dealing with mental health and/or substance use issues
- Families who are under-resourced
- Members of racialized communities.
Why
We Joined
Bramalea CHC is sponsoring an initiative called "WellWork", for prevention of occupational injury and illness, especially for new-immigrant workers. Work Disability aligns with that initiative from a primary care perspective.
